VigiLanz alternatives and competitors for antimicrobial stewardship, AUR reporting and antibiograms
VigiLanz is sold by Inovalon in three hospital tiers now, and the antibiogram sits in the top one. Most teams shopping for an alternative are a small hospital on the Starter tier, a Pro customer at renewal, or a pharmacy that lost its Best in KLAS argument in 2026 and has to re-justify the line item.

Short answer
The main VigiLanz alternatives for antimicrobial stewardship are Wolters Kluwer Sentri7, Premier TheraDoc, BD HealthSight Clinical Advisor, and the stewardship and AUR tools built into Epic. All of them interface with your EHR and lab systems, and none of them publishes a price. If what you need from VigiLanz is the cumulative antibiogram rather than the alert queue, Antimicrobe is a narrower alternative: it builds the CLSI M39 style report from your lab's CSV export with no interface, and its plans are priced on the website from $249 a month.
Why hospitals look for a VigiLanz alternative
VigiLanz was an independent clinical surveillance company from 2001 until Inovalon bought it in February 2024. Inovalon now sells it as VigiLanz Pharmacy Surveillance inside its provider cloud, next to revenue cycle, scheduling and infection prevention products. The software itself is well regarded: Inovalon's product page says it has been Best in KLAS for eight years in a row, and KLAS named it first in pharmacy surveillance in 2024. In the 2026 Best in KLAS report the pharmacy surveillance segment went to Sentri7, which is why more renewals are being questioned this year.
The practical reasons to shop around are structural. Inovalon's page now splits the product into Pharmacy Surveillance Starter for small and critical access hospitals, Pharmacy Surveillance for acute care, Pharmacy Surveillance Pro with advanced analytics, and a separate Ambulatory Surveillance product. The antibiogram is listed under Pro. Inovalon's ONC disclosure describes the fee structure as a one-time implementation fee plus an annual license, with no amounts published. A pharmacy director who bought Starter for AUR reporting and now needs the antibiogram is looking at a tier upgrade and a new quote, with nothing public to benchmark it against.

The VigiLanz tiers, and where the antibiogram sits
Understanding the tiers is most of the decision, because the alternative you need depends on which one you are on. Inovalon's May 2026 Starter overview describes a web-based product for small and critical access hospitals that aggregates data from the EHR, lab and pharmacy systems, validates it, and submits AU and AR data to NHSN in the HL7 CDA format. Its dashboards show facility-wide and unit-level metrics such as days of therapy and proportion susceptible. It does not describe a cumulative antibiogram, and it is explicitly positioned as the tier you start on before upgrading.
The base Pharmacy Surveillance tier for acute care adds near real-time alerts, prioritized worklists, rounding tools, and antibiotic alerts for drug-bug mismatches, missed therapies and de-escalation. The Pro overview adds opioid stewardship, IV-to-PO conversion tracking, medication reconciliation, alerts delivered inside the EHR, and, in Inovalon's words, analytics and antibiograms to visualize usage, resistance and outcomes. So a hospital on Starter or the base tier that wants the antibiogram from VigiLanz is buying Pro.

Sentri7, if you want the current KLAS leader
Sentri7 is the most direct like-for-like replacement. Wolters Kluwer's stewardship page describes real-time alerts, IV-to-oral tracking, SAAR benchmarking, one-click NHSN AUR submission that is NHSN-validated and ONC-certified, and an antibiogram produced from its dashboard. It took both the pharmacy surveillance and the infection control and monitoring segments in the 2026 Best in KLAS report. Like VigiLanz it is one module of a broader platform, called SoleSource, with drug diversion, infection prevention and opioid stewardship alongside, so a hospital that only uses stewardship should ask for single-module pricing before comparing quotes.

TheraDoc and BD HealthSight, if the platform matters more than the tier
Premier's clinical surveillance product, powered by TheraDoc, leads on real-time stewardship alerts, automated NHSN reporting tools, EHR-interfaced data, and coverage of both acute and post-acute settings. That post-acute coverage is the clearest reason to prefer it for a health system that also runs skilled nursing beds. The product page does not mention an antibiogram, so ask to see one printed, with its de-duplication rule and isolate threshold, before assuming it replaces the VigiLanz Pro report.
BD HealthSight Clinical Advisor is described as house-wide antimicrobial stewardship using data from the EMR, order entry, lab and pathology systems, with a companion Infection Advisor with MedMined Insights for infection surveillance. BD's page talks about comparing regional and national resistance data rather than a facility antibiogram. It is the natural choice where BD dispensing and pharmacy analytics are already in the building.

Staying inside Epic instead
For an Epic hospital the alternative to any third-party surveillance product is building the work in the EHR. Epic's certification page lists Bugsy Infection Control as certified for antimicrobial use and resistance reporting, so leaving VigiLanz does not mean losing AUR. The worklists, alerts and antibiogram are builds, and published hospital experience from Johns Hopkins in 2021 describes that as needing stewardship physicians, pharmacists and Epic IT staff with significant time. The trade is a license fee for analyst hours, and the antibiogram is usually the last item to reach the top of that queue.
KLAS's 2026 report on Epic Bugsy is worth reading before you make that move. As summarized by HealthSystemCIO in February 2026, Bugsy scored 80.6 overall, VigiLanz infection control scored 90.8 and was the third-party product most often paired with Bugsy, and organizations that moved from VigiLanz or BD to Bugsy reported lower satisfaction afterward. Many Epic hospitals keep VigiLanz beside Bugsy for that reason.

Keep VigiLanz Starter and buy the antibiogram separately
Antimicrobe is not a VigiLanz replacement and this page does not present it as one. It does not fire near real-time alerts, does not submit AU or AR data to NHSN, does not track IV-to-PO conversions, and has no opioid or medication reconciliation module. If your pharmacists work the VigiLanz queue every morning, or Starter is submitting your AUR data cleanly, keep it.
What it does is the cumulative antibiogram, built the way CLSI M39 describes. You export finalized susceptibility results from the LIS as a CSV. The builder keeps the first isolate per patient, marks cells under 30 isolates, applies a named breakpoint set, cuts by unit, specimen source and quarter, counts MDRO phenotypes, and ranks empiric options by local percent susceptible. Every rule is printed on the report, which is what a P and T committee asks for and what a dashboard antibiogram often leaves off.
That gives a Starter customer a third option beyond upgrading to Pro or leaving. Keep Starter for NHSN AUR, and cover the antibiogram from the same lab export for a published $249 a month on the Lab plan. A Pro customer at renewal can make the same move in reverse: keep the alerts on whichever platform wins the evaluation and take the annual antibiogram off the platform so it stops depending on an interface.

What to check before you leave VigiLanz
The risk in switching is not the new software. It is the gap between systems. Each of these is worth confirming in writing before notice is given.
- AUR continuity: which system submits AU and AR data to NHSN for each month of the CMS reporting period, with no uncovered months
- Rules: how many custom VigiLanz rules you run, and who rebuilds them in the next system
- Intervention history: whether pharmacist interventions and documentation export in a format the next platform can load
- Add-ons: whether NINJA nephrotoxicity monitoring or DoseMeRx dosing sit on the same contract and need a replacement
- Interfaces: realistic build time for ADT, eMAR, lab and microbiology feeds, in your own IT hours
- Contract: the notice period, the auto-renewal date, and the price of staying on a lower tier
Who should stay on VigiLanz
Stay if your stewardship pharmacists work its alerts daily, if Starter is submitting your AUR data without trouble, if your hospital already runs other Inovalon products and the contract reflects that, or if the Pro antibiogram is used and trusted by your committee. A product that held the KLAS pharmacy surveillance segment for years is a poor thing to replace to fix one report. In that case keep it, and fix the report.

Is VigiLanz Starter enough for a critical access hospital?
For the NHSN AUR requirement, that is what it is built for: Inovalon positions Starter for small and critical access hospitals, with automated AU and AR submission in CDA format and no software installs. For the cumulative antibiogram that CMS and Joint Commission surveyors ask a stewardship program to show, you need either the Pro tier, a regional antibiogram, or a separate reporting tool such as Antimicrobe.
Can we use Antimicrobe alongside VigiLanz?
Yes. Antimicrobe reads a susceptibility CSV from the lab and never touches the EHR, so it runs next to any VigiLanz tier without an interface. Teams keep VigiLanz for alerts and AUR submission, and use Antimicrobe for the cumulative antibiogram, its unit and source cuts, and empiric rankings.
